Detailed Responsibilities
The Medical Coder will review physician notes, discharge summaries, procedure details, investigation reports and other clinical documentation to assign appropriate diagnosis and procedure codes according to the project’s coding system and client guidelines. The role includes identifying incomplete documentation, raising queries through approved channels, maintaining productivity and accuracy targets, protecting patient information and participating in quality feedback sessions. Depending on account requirements, the coder may work on outpatient, inpatient, emergency, specialty or procedural records. The position requires strong reading comprehension, clinical terminology knowledge, careful attention to payer or client rules and willingness to learn updated coding guidance. The candidate must maintain confidentiality and follow information-security requirements at all times.
Candidate Profile
The ideal candidate is analytical, patient with detail and comfortable working with digital records for extended periods. They should be open to feedback, able to meet deadlines and committed to accuracy rather than guesswork. Experience with ICD, CPT, HCPCS or specialty coding workflows will be valued. Fresh certified coders with strong healthcare knowledge may also be considered depending on employer requirements.
